About the Study

The goals of the project are:

1. Better understand diagnostic error experiences from patients, families, and caregivers.  Patients' voices are often left out of existing reporting systems, despite being in the best position to communicate about the diagnostic journey. Because of this, patients' experiences are not being used to identify or to learn from these errors. We are currently recruiting patients, family member, and caregivers to share their diagnostic error stories (misdiagnosis, delayed diagnosis, or missed diagnosis) to help us understand how these errors occurs and their impact.

      

2. Identify and address healthcare system barriers to patient reporting of diagnostic errors. While collecting patient reports of diagnostic errors are important, healthcare systems have to be able to use this information to learn from and prevent future errors. Our team will be meeting with healthcare systems to identify their needs and address barriers to implementing a patient reporting system. 


3. In collaboration with patients, family members and caregivers, create and test a patient-centered reporting system. Patient involvement in design and implementation of the reporting system is important to its usefulness and sustainability. We will develop a reporting system, based on the language and experiences of those who have experienced a diagnostic error.  It will be improved with input from patients and experts in patient safety and informatics, and used to create an electronic prototype for further testing.
